DESCRIPTION:This agency offers a range of services, including public clinics, issuance of birth and death certificates, dental screenings, treatment for perinatal mood and anxiety disorders, teen family planning programs, and WIC applications.

ELIGIBILITY:residents of service area only

DOCUMENTS:WIC: Picture ID/driver license, birth certificate for each child, proof of residence or lease, WIC Medical Referral Form, parent and child's weight and mesurements, immunization records and each child's social security number
All other services: call for details
